You can learn more about us here – in short, we’re a division of The Highland Group, LLC, where we’ve been developing medical and dental websites for over 25 years.

The Form Team custom builds state-of-the-art, HIPAA- and ADA-compliant patient forms in two steps – first as a very friendly patient-facing form, and then again when they’re submitted as a duplicate of the forms the practice was originally using.

Our interaction with EHR systems can be seen in detail here. We offer the ability to upload specific sections of patient intake forms to the practice’s EHR, as well as attaching a PDF that matches the exact format and content of their original intake form.

How we can work together

A partnership with The Form Team will allow you to offer our services to your current and prospective clients under any arrangement you prefer. We’ll contact the practice as your company’s ‘form team’, and design, proof and configure whatever intake forms they use, and then integrate them into your system.

We’ll bill you directly for our service (you can see our standard pricing here) – it’s as simple as $99 per page for development and $24 per month for hosting and unlimited support (along with a small potential per-submittal fee if we need to use a 3rd party service to access your system). You can pass this along to your client, absorb it, or mark it up - your choice, but we believe you can certainly advertise it as a definite upgrade that no other EHR system provides.
