Here’s a very simple flowchart of the PracticePlus procedure. You can click on any form image below to see the full-size version of each form.

The Patient Form

Your patient completes, signs and submits the form as usual.

The Submitted Form

Your practice receives the patient’s form, displayed with all the fields (in red) that your doctors/staff can complete. They then sign and resubmit the form.

The Final Form

Your practice receives the final form, containing all the patient’s entries and all your doctor’s/staff’s entries, in the usual Form Team PDF format.
