Our EHR Integration system has all the features of our regular Form Team forms, including:

• The program is a complete turn-key system, hosted on our secure server.
• All setup fees are one-time fees.  No additional charges for updates or changes, ever.
• Monthly hosting fee includes unlimited phone and email support
• No long-term contract – cancel whenever you wish

Because EHR Integration requires us to use a comprehensive third-party system to create and upload your form data to your EHR system, we have to pass on the transaction fee that results in each upload. Fortunately, that transaction fee is only 90¢ for each form submission!
